New patients
Registration is only necessary for our General Practitioners (GP) services. For all other services, you do not need to be registered.
* Please note that you can only be registered with ONE GP in the Netherlands. In the below link- Registration with our General Practitioners- you can read more about why.
You can join the IHCH by completing our registration form.
Once your registration form has been received, you will automatically be sent a medical intake form. Please complete this and bring it with you to your first appointment, together with all your previous medical records.
If you would like to visit the centre or meet our doctors before making the decision to join us, please indicate this to reception staff when making your first appointment.
